community raises multiple concerns.
The number of abortions has increased in the recent period of women, and this increase has caused great concern among husbands, especially newlyweds, because the wife has a repeated abortion process, which results in fatigue to health and fear of repeated pregnancy again, and the multiple causes of miscarriage between the forced and sometimes contrived by the pregnant woman .
"Riyadh" discussed the increase in abortion cases and the reasons leading to it, and the consequent damages after the operation with a gynecology and obstetric consultant, and the ruling on abortion.
Before the sixth
Dr. Issa bin Ahmed Al-Khatimi, a consultant obstetrician-gynecologist at the Military Hospital in Riyadh, explained that abortion means the fetus or part of it leaving the womb before the end of the sixth month (24 weeks ago), that is, before the fetus becomes able to live outside the womb, indicating that abortion is one of the most important causes To vaginal bleeding during pregnancy.
And Dr. Al-Khatimi indicated that abortion is classified into three categories
٭ Medical abortion: It is the termination of pregnancy when there is a risk of pregnancy to the life of the mother, and diseases that may call for abortion include heart and kidney disease and cancerous tumors.
Artificial miscarriage: It is the termination of pregnancy without a legal excuse and without medical reasons, such as the desire not to have children or to prevent the continuing incest of pregnancy and others. This occurs using popular prescriptions without consulting a doctor or by ing substances into the cervix and vagina that lead to miscarriage. Such things may lead to complications and side effects and may lead to death. The work of this type of abortion occurs in unhealthy stores or contaminated devices or under the supervision of those who have no knowledge and experience of this thing.
spontaneous abortion: It is a miscarriage that occurs on its own without any external intervention. This type is one of the most common types that causes bleeding during pregnancy. The percentage of pregnancies ending in a spontaneous abortion in the first months of pregnancy is between 15% to 20%.
Dr. Al-Khatami believes that the incidence of spontaneous abortions is much higher than this percentage, indicating that some pregnancies end in abortion without the woman knowing that she is pregnant.
Types of abortion
And fall under these categories a number of types, the most important of which are
Prognostic abortion: It is a warning of the occurrence of miscarriage and the fetus is still alive and its symptoms are vaginal bleeding with slight pain in the lower abdomen and back and upon medical examination it is clear that the cervix is closed and it is clear through sound rays that the fetus’s pulses are present and clear. About 50% to 60% of these cases, pregnancy continues and the bleeding stops without any complications, whether on the mother or the fetus. To treat this type of abortion, the patient needs rest, not stress, avoiding long-distance travel and refraining from sexual intercourse during this period.
Inevitable miscarriage: It is a miscarriage that must occur and begins with profuse vaginal blood flow accompanied by contractions and pain in the lower abdomen as a result of contractions of the uterus and upon medical examination it becomes clear the widening and swelling of the cervix. To treat this type, the patient is hospitalized, the contents of the uterus are emptied, the patient is given medications to increase uterine contractions and expel the residue of the fetus and the placenta, and a curettage of the uterus may be required.
Incomplete abortion: It is an abortion in which part of the fetus has emerged outside the uterus and parts of it have been left. Upon medical examination, it becomes clear that the cervix is wide and open, and part of the fetus or placenta is outside through it. It is treated by emptying the uterus contents under anesthesia and giving medications that increase contractions of the uterine muscles.
Complete abortion: This is the miscarriage in which the fetus and the contents of the pregnancy have left the uterus completely and the amount of blood is small and the pain stops. Upon the medical examination it becomes clear that the cervix is closed and the size of the uterus is small. This type of abortion does not require treatment or surgical intervention.
المت Aborted abortion: This is the abortion in which the fertilized egg or the fetus died. The fetus may remain inside the womb for a while until the patient begins symptoms of miscarriage, such as bleeding and pain. The uterus also stops growing, and usually this bleeding is in the form of brown vaginal discharge and tends to black. By ultrasound examination, the fetus’s death becomes clear and the heartbeat stops. This type is treated by emptying the uterus of its contents by the process of expansion and curettage.
Recurrent miscarriage: Miscarriage is repeated if it occurs three or more times in a row and the reason for this type may be due to a birth defect of the uterus or the presence of a large fibrous tumor or cervical weakness or a lack of pregnancy hormones and sometimes the cause is unknown.
Moldy abortion: This type occurs as a result of inflammation of the uterus, tubes, ovaries and pelvis as a result of germs entering the vagina after the abortion. This condition may occur in all cases of spontaneous and medical abortion, but it most often occurs with artificial (criminal) abortion. The patient will experience a rise in body temperature, headache, vomiting, severe stomach discomfort, and foul-smelling vaginal discharge. To treat this type, the patient must be hospitalized and begin administering antibiotics through a vein, analgesics and fluids until the patient''s condition has stabilized, and then the uterus is emptied of its contents.
Causes of miscarriage
Dr. Issa added that abortion occurs sometimes as a result of diseases affecting the fetus and its underdevelopment, and in other times to diseases related to the womb of the mother, but in many cases abortion occurs without known causes, the most important of which are:
birth defects in the formation of the fetus and this reason is considered one of the most common reasons, as it represents between 50% to 80% of all cases and occurs as a result of an imbalance in the fertilized egg or birth defects in the formation of the fetus or hereditary dyes.
Lack of hormones necessary for the continuation of pregnancy. In the first months, pregnancy requires the hormone progesterone which is excreted from the ovary to continue the pregnancy. After the first three months, the placenta begins to secrete this hormone until the end of pregnancy. And the lack of this hormone leads to early abortion. When a woman has a miscarriage, and it is suspected that this is due to a lack of this hormone, the patient is given this hormone during the first three months to help continue the pregnancy, God willing, naturally.
Congenital defects of the uterus, these defects may be acquired or congenital malformations. Examples of congenital anomalies are the uterus, the uterus of one century, or the presence of a barrier inside the uterine cavity, which leads to premature births or may lead to miscarriage. Another reason is the presence of fibroids on the uterine wall, especially when they are large, which may lead to miscarriage.
Cervical dysfunction is either due to a birth defect or some surgeries of the cervix, where the pregnancy does not continue due to this weakness and the opening of the cervix is easy. This weakness is treated by tying the cervix after the completion of the first three months or at the beginning of the fourth month under general anesthesia. As for its removal, it does not require anesthesia and is carried out at the beginning of the ninth month.
Endocrine diseases, such as thyroid disease, either increase or decrease in its activity, as well as diabetes.
High body temperature during pregnancy due to some diseases such as appendicitis, urinary tract infections, pulmonary infections, pelvic and others that may lead to miscarriage and such cases should early intervention in order not to exacerbate the disease and in order to reduce the occurrence of miscarriage, God willing.
Infectious diseases during pregnancy, such as some viruses, such as rubella, regular measles and the herpes virus, may lead to miscarriage. In addition, infection with these viruses in the first months may lead to infection of the fetus with deformities, and thus miscarriage. Another example is pregnancy infection with (Toxoplasmosis), which is called cat disease (Toxoplasmosis), which affects the fetus and leads to miscarriage and toxoplasma is a disease caused by a type of parasite that lives in cats and some other animals and these parasites are transmitted to humans by eating contaminated raw meat or in contact with Cats and their breeding Therefore, the pregnant woman should not be exposed to cats and raising them and avoid eating raw meat.
Pregnancy exposure to some types of radiation and a large amount, especially during the first months of pregnancy, may lead to fetal deformity, preferably avoiding any radiological examinations in the first three months, and informing the doctor or radiologist that you are pregnant or delaying your period and suspecting pregnancy.
Its serious ??
And Dr. Al-Khatimi warned of complications that may result from abortion because it is considered one of the causes leading to the death of mothers as a result of severe bleeding or infections of the uterus and pelvis, which leads to anemia due to this bleeding and the process of curettage and expansion may lead to some complications, especially if it is not done under trained hands . This is with regard to the complications that accompany the abortion process. As for the complications that follow this process, if the mother''s blood type (negative), it is necessary to give her a single injection of anti-agent (Aunti-D Gobulin) within 72 hours after any type of abortion, after the abortion comes The previous cycle, and pregnancy may occur after the abortion before the onset of the next cycle if contact with the husband occurred during this period, therefore care must be taken and the use of appropriate contraceptive methods, after the abortion the patient must see a doctor when these symptoms occur high temperature or severe and persistent pain In the lower abdomen, general weakness, fainting, dizziness and vaginal discharge with an unpleasant odor or continuous bleeding more than three weeks or heavy bleeding for three days or more, and finally not to fall during the six-month period after the abortion.
Islamic ruling
Sheikh Zaid bin Omair Al-Shammari, a judge at the General Court in Riyadh, stressed the importance of this matter, which is worthy of research and care from medical and Islamic scholars, and added:
Abortion has become commonplace in most countries of the world in the modern era, for reasons that are mostly prohibited and some of it is necessary. Also, he has absolutely permitted it.
Abortion is of two types
Sheikh Al-Shammari explained that abortion is of two types
The first: Compulsive abortion: It is that which occurs without external interference and neither the woman nor anyone else will have it. This is not described as a solution or a sanctity. Because without the will or intent of women and legal rulings, it is based on what happened with the will and intent, if the intentions are considered in all actions and things are based on their intentions.
The second: voluntary abortion: It is the deliberate removal of pregnancy from the womb in a way other than its usual date by any means and it is not livable, and the legal ruling for this type varies according to the reasons for it
