"obstetrics and gynecology", and there is a global reluctance to do so
Head of the Organizing Committee for Clinical Risk Management in Obstetrics and Gynecology, which is to be implemented by Security Forces Hospital, on Monday, Dr. Hessa Saleh Al-Duhami, confirmed that the idea was taken from the large number of medical errors, of which about 22% of them were detected in obstetrics and gynecology. She said in an interview with "Al-Riyadh" that 12 cases of medical error will be revealed in this conference after the courts dealt with them. She explained that some of these cases ended with a hysterectomy and another for a dead child, a case for a mother who had blood flow and head haemorrhage, and another case suffered from blood clotting and a clot. She pointed out that the goal of holding it on Monday, Shawwal 24, at the Security Forces Hospital in Riyadh, is to prepare a plan to study the risks, and to develop solutions before they happen as much as possible. Referring to the Security Forces Hospital, which has a good experience, it helped in reducing the negative aspects and avoiding the risks that the mother or her fetus may be exposed to.
Text of the dialogue
How did the idea for this conference come about?
It came after about 22 percent of medical issues related to obstetrics and gynecology were detected.
What is the real purpose of it?
Preparing a plan to study risks, and develop solutions before they happen as much as possible, and we have at the Security Forces Hospital this department for about a year and a half and helped in reducing the negative aspects and avoiding the risks that may encounter the mother or her fetus, and the Women and Childbirth Unit has achieved tangible results, thank God, and most medical issues What goes to the courts is the disability of children, so we look at how many doctors cause these disabilities? What are its causes? There are sometimes 80% of these issues that are not a medical error as much as they are a mistake in communication in the sense that the doctor, for example, entering a nervous situation or the nurse treating him in a bad way, and this occurs a problem of the nature of the disease and not due to a medical error and therefore this is called a communication error between the doctor and the patient.
Who are the speakers?
Dr. Helen is from Britain, and there are participants and speakers from Bahrain, Kuwait and Qatar. Dr. Helen will review the British system for risk management, and will assist in developing appropriate plans according to the circumstances of each hospital, and who are the management members? How do you study the cases? And where are the places that should be focused on in the hospital sites, for example, is it emergency or operations, etc. ?, and thus will help us establish a clinical risk management within the hospital, in addition to that there is a legal lawyer who will talk about legal matters, whether with doctors, nursing staff, anesthesia, or any specialty.
How many participants?
We expect 350 doctors in obstetrics and gynecology specialties, and so on. On the first day, a discussion of risk management in hospitals and criminal cases will be discussed, and some Sharia cases considered by the Saudi courts, where Dr. Khalifa Al-Melhem, Director General of Health Affairs in Al-Baha, participated and will raise some of the cases that he entered as an investigator and where the errors were And there is Dr. Abdul Hamid Samarkandi, a consultant anesthesiologist at King Khalid University Hospital, who is from the Sharia court, and he will talk about his experience and the cases that have been recorded in cases of medical errors, and Dr. Yasser Al-Obaida is participating about how to a safe environment for patients in the health system in the Kingdom, and there is a star from the Security Forces Hospital who will talk about mistakes Morality which falls in the specialty of women and childbirth in the light of Sharia courts On the second day, Dr. Helen from Britain will discuss the components of risk management in detail and how to apply. The conference will present the experience of Kuwait, Qatar and Bahrain in this matter, in addition to medical lectures for attendees.
What is the expected percentage after establishing risk management in reducing medical errors rates?
If we achieve 30% in the first year, it is good, but our target is 80% and it is difficult to obtain it, and therefore a time plan must be developed to reduce these errors.
How do you see the spread of medical errors in the Kingdom''s hospitals?
There are many, but there is no recording of all errors, in Britain you find that there is a center that collects and records all cases, and every hospital keeps its statistics, and this is for large hospitals in Riyadh, Jeddah and Dammam, but in other cities and villages you do not know the causes of any harm to the patient and whether what the patient was exposed to is a mistake Medical or not....
The large number of medical errors in women and childbirth. To what extent is this statement correct?
This is true, because it has a special nature, as most cases reach the hospital in emergency situations, and doctors in this specialty are few, and some hospitals suffer from that.
What is the percentage of medical errors that occur on women and childbirth out of the total errors recorded in the Kingdom?
I expect that there is an approximate percentage through my work and experience, which is that 40% of medical errors occur in women at the moment of childbirth, and errors are deaths and disabilities for fetuses such as mental or physical disabilities, hemorrhage or maternal deaths.
Why are these mistakes so many?
The reasons are many, but there may be a slow decision-making for the patient, for example, a cesarean section was required, and with delay, mental disability of the fetus and other such cases occur.
Will they discuss issues that reached the courts, and what is their purpose?
Yes, we will discuss some issues to know their causes precisely and to know the defects in a specific scientific manner, and the number of 12 cases are medical errors, the most prominent of which are cases of hysterectomy, a case of a dead child, a case of a mother who was afflicted with blood flow and a head haemorrhage, and another case of blood clotting and a clot.
Can you say that medical errors are constantly increasing? Why?
Yes, it is increasing, and the fear of issues has led some private sector doctors to tend to caesarean section. In America, the percentage of caesarean section reached 40% of all births annually in some of our hospitals in the Kingdom, and it reached about 35%.
Difficulty getting statistics on medical errors, why?
Yes ... we have a problem in the process of communication between health authorities and the exchange of statistical information, which is weak and there is bureaucracy and fear, and this should never be if we take our aim is to correct the error.
Is there a big bug in that?
Yes, there is a defect in the system. In Canada, Britain, and other countries they have a guideline for clinical treatment and approved, for example, a diabetic patient must be diagnosed in such-and-such way and his treatment includes such-and-such, and so-and-so complications must be prevented, and therefore everything is clear. These guidelines expose themselves to accountability, and these guidelines were not put in place without interest. Rather, there are committees that stay for months to study the best for the patient, and here there is no advisory system and some hospitals have that, and there is a question, who is responsible for giving treatment advice. Is it the Ministry of Health? Or other bodies are specialized associations, and there is a disparity in the doctors'' learning about the system while studying in Canada, Britain or Germany, and it s a kind of defect.
How many births do we have in the Kingdom and how many deaths are there?
Approximately between 450 and 500 thousand newborns per year, and the mortality rate reaches a thousand and reaches 12 per thousand, which means 1.2%. There are causes of death and the reason is that the child was born premature or has congenital anomalies and this reaches 80% and there are other reasons such as medical errors.
How do you see children about the specialization of women and childbirth of both sexes, especially women?
There is a reluctance recently, and the obstetrics and gynecology department has decreased the desire of people to it globally, and the reason is the large number of medical problems, and in Canada medical insurance for obstetricians and gynecologists reaches 800 times compared to other doctors, and there is a demand for other specialties that have a financial return, such as plastic surgery, ophthalmology, and others.
