treatment of keloids and hypertrophic scars?
Keloids and hypertrophic scars are one of the most important problems
And the dilemmas facing doctors and their patients. Especially after wound healing and surgeries, the poor appearance of these scars may cause great dissatisfaction in many patients.
Keloids and hypertrophic scars usually occur in puberty and during pregnancy, and it also increases in some families. It may infect anywhere on the surface of the skin. However, there are some places on the surface of our skin where the formation of such scars is higher than others, and these places are the shoulders, upper back, front of the chest and neck.
Among the well-known and popular treatments for these scars: surgical excision, radiation therapy, laser, pressure therapy, cortisone injections, cold cautery, and application of silicone compounds.
The ease of applying silicone compounds as an easy procedure that does not require surgical interventions has led to the widespread spread of these compounds, and it has started to be marketed locally since 1990 and there are now more than 60 compounds in the global market, either in the form of stickers or gels, foam, Liquid, spray, and ointment.
The primary effect of silicon compounds in reducing the size and flattening of these scars is not fully known. It has been believed that the hydration process that occurs as a result of applying silicone stickers for a long period of time increases oxygen retention, and it is believed that this retention has an immunological role in inhibiting the size of scars.
Another theory attributes the effect to the high temperature when applying these patches, thus increasing the effect of the enzyme that destroys collagen. It is known that collagen is mainly formed from a group of fibroblasts called a fibroblast. These cells are the nucleus in the formation of stigmata and keloids.
Many attempts and studies have been made to prove the effectiveness of these patches, and most studies have shown that these patches lead to a change in the thickness, color and texture of these scars. And some companies went to add vitamin E and cortisone with silicon to get better results. Despite those good and acceptable results that some scientists and researchers have observed in using these silicone pads, but we must not exaggerate their usefulness, because there are some other studies that have shown that topical injection of cortisone into keloids and scars definitely gives better results than silicone patches.
