treatment.

It is required to know the following as a culture for both spouses: • Natural fertilization: * Natural fertilization results from the meeting of two cells. The female sex cell or egg. And the male sex cell or the spermatozoon in a way that leads to the formation of the fetus later. This convergence does not occur unless there are a number of circumstances represented by the periodic characteristic of the woman''s ovarian activity, so in contrast to the man, the woman''s ovary has a cyclical activity where ovulation (i.e. the release of the egg from the ovary) occurs in the middle of the cycle and menstrual blood comes out of the uterus at the end of the cycle if pregnancy does not occur.

important note

1- The cycle (which is the time period between two menstrual cycles) and is calculated from the first day of menstrual bleeding to the first day of the next menstrual bleeding, and this period usually ranges between 28 to 30 days and the fertilization period is between the twelfth to the sixteenth day of the cycle when Women whose menstrual cycle is like this.

Thus, when we talk about fertilization rate, we mean the possibility of fertilization and thus pregnancy "for each menstrual cycle".

2- Low fertility

Research has indicated that the fertilization rate among healthy couples is not 100%, but only 25% per menstrual cycle.

Consequently, society is divided into several categories for statistical purposes only

• A couple more fertile and have a high chance of a natural pregnancy, that is, up to 50% for each menstrual cycle (25-50%).

• Couples who are less fertile and have a low chance of a normal pregnancy, between 1-20% for each menstrual cycle.

In very few cases, the chance of a natural pregnancy is less than that, and this is called difficulty fertilization. So, low fertility or difficulty fertilization can be defined as failure to conceive during a regular marital relationship for a period of one to two years.

Various reasons may affect normal fertilization

Age of the woman: the peak fertility capacity of a woman between the age of 20-30 years, but after the age of 35 years, the ability to fertilize begins to gradually diminish.

Marital sexual intercourse: The previous figures regarding fertility in husbands, which we mentioned are in the normal range, in the presence of regular marital intercourse. In fact, to talk about fertilization, marital encounter is assumed to be during the woman''s fertilization period (from day 12 to approximately 16).

So the chances of pregnancy are sequential by the number of menstrual cycles a woman has, and the factor of "time" here means the number of menstrual cycles in which there was a regular convergence during the fertilization period between the spouses, and only then can one judge the ability of the spouses to natural fertilization.

Thus

Young spouses who are normal and free from certain health influences, and in light of the meeting between my spouses who have regulated the fertilization period, the incidence of pregnancy is 95% within a year, i.e. 12 menstrual cycles - On the other hand:

If the spouses are less fertile (from 1-20% per cycle), for example, the expected fertility is 10% per menstrual cycle, and in light of regular marital convergence during the fertilization period, the rate of pregnancy is 70% during 12 menstrual cycles.

So, there is no need to worry and search for the reasons for not getting pregnant before the passage of 12 menstrual cycles, from the beginning of life, the desire to conceive in the presence of a regular meeting between the spouses, and therefore we see that some infertility treatment doctors do not begin to search for the reasons why pregnancy does not occur before the passage of a year and modern recommendations extend The period to two years. In any case, if the woman exceeds the age of thirty-five or there are previously known reasons that may affect the man’s fertilization, it is preferable to start the search and examination after a shorter period.

From whom we take advice

In many societies, and our society may be one of which is the rapid inability to conceive, it is unfortunately considered a sin or calamity, and therefore everyone gives his opinion and everyone wants to extend a helping hand and advice abounds and rumors start and here the couple is affected psychologically and intellectually and a problem begins in terms of nothing, it may be the period of 12 cycles Naturalness has not yet passed, or the regular marital meeting may not have taken place for travel or other reasons.

And here begins thinking about folk medicine in its various names, and the difficulty of meeting a doctor or fear of examinations and going to hospitals may facilitate the process of resorting to folk medicine, which has no discipline in its practice, and here the disaster may be so that the state of poor fertility worsens, but there may be no weakness and then this weakness arises in Fertilization as a complication of treatment with folk medicine for those who do not know it.

Then there is prophetic medicine and reading in the Qur’an with our complete belief and our complete belief in everything from God and His Messenger, may God bless him and grant him peace. But seeking treatment with specialist doctors after the previously specified period is what is required and all of that is knowledge from God who taught us it.

Therefore, going to a doctor specialized in treating difficult infertile diseases, or before that, to a family doctor, if available, is what is required to take advice and follow-up before requesting it from any other source.

Who needs medical advice from both spouses?

In fact, it is desirable for both of them to go to medical advice at the same time, as the fertilization process is a joint product of both parties, so the efficiency of the egg and the efficiency of the two sperms are required together to obtain the desired fertilization. The discovery of deficiencies and achieving the required result is the result of a joint examination by both parties, whether at the level of vital laboratory analyzes or proper planning for future treatment stages, whether deficiencies in one of the parties or both.

If, as mentioned, and since the treatment is for both parties, it is preferable that the spouses visit the doctor together at least for the first time.

There may be some embarrassing questions for the wife or husband, but we think that it is very important for them to be together, but this may be the beginning of the complete integration between the spouses and the correct understanding of the problem. And join hands to confront it and work to solve it with the help of God and then with the help of the doctor.
