A girl 6 years old came to KSMC , ER complaining of trauma by pencil that enters through the RT. upper lid into the brain . The patient admitted as case of orbital cellulitis because the on admission has fever ,NLP vision of RT.eye ,severe lid edema,frozen globe ,dilated non-reactive pupil and proptosis , but CT scan showed foreign body compressing the optic nerve and enters the brain through the optic canal . The was seen by neurosurgeon who decided to take the patient to OR to remove the foreign body.

The first follow up visit , the patient vision still NLP of RT. eye , complete ptosis, no proptosis , dilated non-reactive pupil, frozen globe and pale disc . The patient referred to neurology for assessment of cranial nerves and more follow ups.Post-op CT scan
